The following conditions and behaviors may indicate a patient is appropriate for hospice evaluation:
- Frequent re-hospitalizations
- Progressive decline in cognitive abilities
- Progressive decline in functional abilities
- Increase in medications
- Incontinence of bladder and bowel
- Unintended weight loss/gain
- Refusal to eat
- Loss of ability/difficulty to swallow
- Loss of ability to sit up without assistance
- Loss of ability to walk independently
- Frequent infections
- Non-healing wounds/increasing number of wounds
- No longer responding to treatments
- No longer desiring treatments
